Use this form to submit a ride proposal to the ride coordinator. Please make sure that you are logged in first before using this form and that you have a “Ride Leader” role. Check with the webmaster to set that up, if appropriate. For reference your “role” is shown on your member profile page (Member Profile Page).
Ride leaders must familiarize themselves with these instructions explaining how to handle an accident on their ride, and must always carry a copy of the LAB accident report form on the ride (see link to it below).

Ride leaders should use this form to keep track of participating riders on road rides. Front side is for club members. Reverse side is for guests.
Anyone under 18 participating in a club road ride must have a signed release form. If you are expecting minors on your ride, make sure you have these forms available.
Ride leaders should use this form to keep track of participating riders on mountain bike rides. Club mountain bike rides are members-only, so there is no spot for guest signup. This is all dictated by our insurance company (LAB).
Our insurance company (LAB) now requires all riders on a club organized mountain bike ride to sign a waiver (release form) at the start of each ride. So we can’t rely on the waiver that each member has signed to be a member of the club. So Mountain Bike Ride Leaders should have a set of these forms available for each ride to cover all the riders they expect on the ride.
Ride leaders should always carry a copy so that all incident details can be recorded at the site. The form includes a short list of instructions explaining what must be done on the ride. Complete instructions, including what to do after the ride, can be found on the Accident Reporting Webpage.
